The best approach is a standard three‑view wrist series: posteroanterior (PA), lateral and oblique. If a scaphoid fracture is suspected, add an ulnar‑deviation PA or dedicated scaphoid view to improve fracture detection. This three‑view series is the standard initial imaging.

The best view is the dedicated scaphoid projection: a PA wrist with maximal ulnar deviation and a 10–30° cranial (toward elbow) x‑ray tube angulation. This elongates the scaphoid and minimizes foreshortening, improving fracture visualization. Standard PA/AP, oblique and lateral views are useful adjuncts, but the PA ulnar‑deviation (scaphoid) view is most sensitive for detecting scaphoid fractures.
