AP (anteroposterior) and lateral are common X‑ray projections. AP means the X‑ray beam passes from front (anterior) to back (posterior) — often used when patients can’t stand; it can magnify structures like the heart. Lateral view is taken from the side, with beam passing right-to-left or left-to-right, providing depth information, clarifying overlap, and localizing lesions or assessing bone and joint alignment.

A normal lateral elbow X‑ray shows a true lateral projection with humeral condyles superimposed, intact cortical margins and preserved joint spaces. The anterior humeral line intersects the middle third of the capitulum and the radiocapitellar line passes through the capitellar center. A small anterior fat pad may be visible; the posterior fat pad is not seen. No displaced fractures, subluxation, or effusion are present.

In an AP elbow radiograph the patient’s arm is extended (or slightly flexed if painful) with the forearm supinated so the palm faces upward. The humerus and forearm are aligned and the medial and lateral epicondyles are parallel to the image receptor. The X‑ray beam is centered at the elbow joint to include distal humerus and proximal radius and ulna for a true AP view.

Obtain AP and lateral elbow radiographs. For AP: patient seated or supine, arm extended, palm up (supinated), humerus and forearm on same plane; center the beam perpendicular to the mid‑elbow to include distal humerus and proximal forearm. For lateral: elbow flexed 90°, medial side on the detector, thumb up (true lateral); center beam perpendicular to the joint. Immobilize painful limbs, use shielding and collimate.
