An AP hip X‑ray is typically performed with about 15 degrees internal rotation of the leg (often cited as 15–20°). This aligns the femoral neck parallel to the image receptor, reducing foreshortening and improving visualization of the femoral head and neck. Care should be taken to position the pelvis symmetrically and avoid excessive rotation that can distort anatomy.

An X‑ray pelvis with both hip PA view is a radiographic exam imaging the pelvic bones and both hip joints using a posteroanterior (PA) projection. The X‑ray beam passes from the back toward the front to capture bone alignment, joint space, fractures, dislocations, arthritis or infection. It’s ordered for trauma, persistent hip or groin pain, limited mobility, or pre/postoperative assessment.

A pelvic and hip X-ray visualizes the pelvic bones, hip joints, femoral heads and necks, and surrounding joint spaces. It detects fractures, dislocations, arthritis and joint-space narrowing, degenerative changes, bone lesions or tumors, avascular necrosis, congenital abnormalities and implant position. It also shows bone alignment and soft-tissue calcifications, guiding diagnosis and treatment planning.

On an AP pelvic radiograph the legs are internally rotated about 15–20° to bring the femoral necks parallel to the imaging receptor. This reduces foreshortening and superimposition, providing truer neck length and alignment and better visualization of the proximal femur for fracture or deformity assessment. If a hip fracture is suspected, rotation may be omitted to avoid pain or displacement.

Internal rotation of the hip is turning the thigh inward toward the body’s midline at the hip joint, produced by medial rotation of the femur. Primary internal rotator muscles include the anterior gluteus medius and minimus and the tensor fasciae latae. It helps with walking, balance, and athletic movements; reduced internal rotation can cause pain or an abnormal gait.

A doctor orders a pelvis X‑ray to evaluate bone and joint problems in the pelvic region: to detect fractures or dislocations after trauma, assess hip pain or suspected arthritis, look for infection or tumors, check alignment or degenerative changes, and review prostheses or post‑operative healing. It helps guide diagnosis, treatment planning, and whether further imaging (CT or MRI) is needed.
