An AP (anteroposterior) shoulder X-ray projects the X‑ray beam from front to back, usually with the patient standing or supine and the arm in neutral rotation. It shows the clavicle, acromion, glenoid and humeral head, allowing assessment of fractures, dislocations, joint space and alignment. AP views are often paired with axial or scapular‑Y views for fuller evaluation.

An outlet view is a specialized shoulder X-ray projection (also called the supraspinatus or Neer outlet view) that profiles the acromion and coracoacromial arch. It uses an angled beam to visualize the subacromial space, acromial shape, spurs, and scapular neck, helping diagnose impingement, rotator cuff pathology, acromial fractures, and bony morphology contributing to shoulder pain.

The three standard shoulder X‑ray views are: 1) Anteroposterior (AP) view—often obtained in internal and external rotation to assess the glenohumeral joint and humeral head; 2) Scapular Y (lateral) view—to evaluate dislocation and acromion; and 3) Axillary view—helps assess the glenoid rim, humeral head position, and joint space. These three views together improve detection of fractures, dislocations, and arthritis.

A shoulder AP view's X‑ray exposure lasts only fractions of a second; with patient positioning and staff instructions it typically takes about 5–10 minutes. Allow 10–20 minutes for the full visit including registration and any repeat or additional views. More complex cases (e.g., trauma immobilization or special projections) can extend the time.

An outlet (supraspinatus/Neer) X‑ray is a tangential shoulder projection that images the coracoacromial arch and subacromial space. Taken with a cephalocaudal beam angle, it highlights acromial shape, spurs, os acromiale and any narrowing that can cause superior rotator‑cuff impingement. It complements standard shoulder views to evaluate pain, impingement and guide further imaging or treatment.

AP stands for Anteroposterior. In an AP X‑ray the X‑ray beam enters the patient’s anterior (front) surface and exits the posterior (back) surface before reaching the detector. This projection is often used for portable, supine, or immobile patients (e.g., chest or abdominal views). It contrasts with PA (posteroanterior), where the beam passes from back to front.
