Ultrasound Skull

Ultrasound cannot reliably image the brain through an intact adult skull because dense bone blocks sound waves. In infants, open fontanelles allow cranial ultrasound to visualize the brain. In adults, transcranial Doppler can use thin temporal bone "windows" to assess cerebral blood flow but gives limited structural detail. CT or MRI are preferred for detailed skull or brain imaging.

Doctors order a head ultrasound to image the brain noninvasively at the bedside. In newborns it detects bleeding, hydrocephalus, congenital malformations, infections or cysts—especially in premature infants. In older children and adults, transcranial ultrasound/Doppler assesses cerebral blood flow to detect vasospasm, vessel narrowing, emboli risk or monitor stroke. It’s quick, safe and useful for diagnosis and follow-up.

The skull theory claims fetal skull shape on ultrasound predicts sex: rounder, smoother foreheads and smaller chins for girls; more angular foreheads, pronounced brows and square jaws for boys. It’s based on assumed hormonal effects on bone shape but lacks scientific validation and shows low accuracy. Reliable sex determination uses genital visualization on ultrasound or genetic testing.

Head ultrasound (cranial ultrasound in infants and transcranial Doppler in adults) shows brain structure and blood-flow information. In newborns it images ventricles, bleeding, hydrocephalus, cysts, white‑matter injury, congenital abnormalities and infections through the open fontanelle. In adults it assesses cerebral blood flow, vessel narrowing, vasospasm and emboli. It’s noninvasive and bedside-friendly but limited versus CT/MRI.

For evaluating the skull bones and fractures, a CT scan (particularly non-contrast CT) is best because it shows fine bone detail. MRI is superior for soft-tissue, brain, marrow, and tumor assessment, and for vascular evaluation CT angiography or MR angiography may be used. In acute head trauma, non-contrast CT is typically the first choice.

Ultrasound can detect certain brain problems but mainly in infants and for blood-flow assessment. In newborns, cranial ultrasound (through fontanelles) reliably finds hemorrhage, hydrocephalus, cysts, and major malformations. In adults, transcranial Doppler assesses cerebral blood flow, vasospasm and stenosis but cannot image detailed brain structure—CT or MRI are needed for most adult brain pathology.
