Ultrasound Bladder

A bladder ultrasound cannot directly diagnose a urinary tract infection because it cannot detect bacteria. It can, however, identify complications or contributory problems—bladder wall thickening, retained urine (post‑void residual), stones, obstruction, or abscess—that may accompany or predispose to UTI. Ultrasound is useful in recurrent or complicated cases to find underlying causes; definitive diagnosis relies on symptoms plus urinalysis and urine culture.

A full bladder ultrasound scan itself is quick, typically 5 to 15 minutes of imaging. Including registration, positioning, measurements and a brief report, expect about 15 to 30 minutes at the clinic. If you must arrive with a full bladder and need to wait to fill it, add 30 to 60 minutes. Follow any prep instructions so the scan can proceed promptly.

A normal bladder ultrasound shows a smooth, regularly contoured bladder with thin walls when adequately distended (typically ≤3–5 mm), no masses, stones, diverticula, or trabeculation, and complete or near-complete emptying with a low post-void residual (commonly <50–100 mL). Interpretation depends on clinical context and bladder filling.

To prepare for a bladder ultrasound, drink enough water to arrive with a comfortably full bladder—typically 500–1000 ml (about 2–4 cups) 45–60 minutes before the appointment—and do not urinate until after the scan. Wear loose clothing or bring a change; you may need to remove lower garments or wear a gown. For post‑void assessment you’ll be asked to empty your bladder during the visit.

Bladder ultrasound can show stones, tumors or intravesical masses, wall thickening and trabeculation (from chronic outlet obstruction), diverticula, bladder wall inflammation or cystitis, intraluminal blood clots or foreign bodies, urinary retention with increased post‑void residual, congenital anomalies and fistulae, and signs suggesting obstruction such as hydroureteronephrosis. Ultrasound is useful for screening but may require CT, cystoscopy, or urology referral for definitive diagnosis.

Ultrasound cannot directly identify bacteria, but it can detect signs suggestive of bacterial infection—abscesses, fluid collections, tissue swelling, increased blood flow on Doppler, or organ inflammation. It’s useful to locate lesions and guide needle aspiration for microbiological culture and drainage. Limitations include lower sensitivity for early or deep-seated infections; blood tests and cultures are needed for definitive diagnosis.
