Transferrin

Transferrin is a plasma glycoprotein that binds ferric iron (Fe3+) and transports it from absorption sites and macrophages to bone marrow and other tissues. By tightly binding iron it limits free iron’s oxidative toxicity and makes iron available to cells via transferrin‑receptor‑mediated endocytosis. Clinical measures—transferrin, transferrin saturation and total iron‑binding capacity—help evaluate iron status and disorders of iron metabolism.

Low transferrin reduces iron transport and is seen with liver disease, malnutrition, chronic inflammation and nephrotic syndrome. It lowers total iron‑binding capacity and can contribute to anemia by impairing iron delivery to the marrow, causing fatigue, pallor and increased infection risk. Evaluation usually includes iron studies, liver tests and inflammation markers to identify and treat the underlying cause.
