Tacrolimus Genotyping

Tacrolimus genotyping is a genetic test that identifies variants—primarily in CYP3A5 (and sometimes CYP3A4/ABCB1)—that affect metabolism of tacrolimus. Determining whether a patient is a CYP3A5 expresser or non‑expresser helps clinicians predict drug clearance and individualize starting doses to reduce rejection risk and minimize toxicity, especially in organ transplant recipients. Results guide monitoring and dose adjustments.

The genetic test for tacrolimus is a pharmacogenetic assay that detects CYP3A5 variants (most commonly CYP3A5*1 and *3) and sometimes CYP3A4/ABCB1 polymorphisms. Using DNA from blood or a cheek swab, it determines CYP3A5 expression status (expressor vs non‑expressor) to guide initial tacrolimus dosing, helping achieve target blood levels more quickly and reduce rejection or toxicity risk.

The tacrolimus test measures blood levels of the immunosuppressant tacrolimus to guide dosing, prevent organ rejection and avoid toxicity. It’s used for therapeutic drug monitoring in transplant recipients and some autoimmune conditions, after dose changes or drug interactions, or when toxicity or nonadherence is suspected. Results (usually trough levels drawn before the next dose) help clinicians adjust therapy safely.

Tacrolimus is an immunosuppressant that binds FKBP‑12 and inhibits calcineurin, blocking T‑cell activation and cytokine (notably IL‑2) production. By dampening immune responses it prevents organ transplant rejection and treats inflammatory skin conditions such as severe atopic dermatitis. It’s used systemically for transplant patients and topically for certain skin diseases, with monitoring for infection risk and drug interactions.

Genotyping tests identify specific DNA variants—such as single nucleotide polymorphisms (SNPs), small insertions/deletions and alleles—in a person’s genome. They detect inherited mutations and carrier status, assess genetic risk factors for diseases, reveal pharmacogenetic markers that influence drug response and dosing, and can characterize tumor mutations. Results help with diagnosis, risk assessment, treatment planning and familial genetic counseling.

Tacrolimus doesn't directly cause cancer, but long-term immunosuppression raises risk of several malignancies. Most commonly non-melanoma skin cancers (squamous and basal cell carcinoma), post-transplant lymphoproliferative disorders including non-Hodgkin lymphoma, and Kaposi sarcoma. There’s also a modestly increased risk of other solid-organ cancers (lung, kidney, liver, and anogenital cancers). Regular screening and sun protection are recommended.
