Determining whether a patient is a CYP3A5 expresser or non‑expresser helps clinicians predict drug clearance and individualize starting doses to reduce rejection risk and minimize toxicity, especially in organ transplant recipients. Results guide monitoring and dose adjustments.

The genetic test for tacrolimus is a pharmacogenetic assay that detects CYP3A5 variants (most commonly CYP3A5*1 and *3) and sometimes CYP3A4/ABCB1 polymorphisms. Using DNA from blood or a cheek swab, it determines CYP3A5 expression status (expressor vs non‑expressor) to guide initial tacrolimus dosing, helping achieve target blood levels more quickly and reduce rejection or toxicity risk.

Tacrolimus doesn't directly cause cancer, but long-term immunosuppression raises risk of several malignancies. Most commonly non-melanoma skin cancers (squamous and basal cell carcinoma), post-transplant lymphoproliferative disorders including non-Hodgkin lymphoma, and Kaposi sarcoma. There’s also a modestly increased risk of other solid-organ cancers (lung, kidney, liver, and anogenital cancers). Regular screening and sun protection are recommended.
