Stress MPI Test

An MPI (myocardial perfusion imaging) stress test is a noninvasive nuclear scan that evaluates blood flow to the heart muscle at rest and during stress. A small radioactive tracer is injected, and images are taken after exercise or a medication that simulates exercise. It detects areas of reduced blood flow, assesses coronary artery disease and prior damage, and helps guide treatment; risks are low but include mild allergic reactions or small radiation exposure.

Costs vary widely by country and setting. In India, a stress myocardial perfusion imaging (MPI) scan typically costs about ₹3,000–₹12,000; in the UK it’s free on the NHS or roughly £300–£900 privately; in the US expect about $1,000–$3,000 (hospital prices can be higher). Insurance, location and inpatient vs outpatient status affect the final price.

Myocardial perfusion imaging (MPI) stress testing can cause short-lived side effects: chest discomfort, palpitations, shortness of breath, dizziness or lightheadedness, nausea, headache, flushing, and injection-site soreness. Pharmacologic agents used for stress may cause flushing, breathlessness, bronchospasm (in people with lung disease), low blood pressure or arrhythmias. Allergic reactions and rare serious cardiac events are uncommon. Small radiation exposure occurs from the tracer.

A two-day nuclear stress test separates rest and stress imaging on different days to improve image quality and diagnostic accuracy. It’s chosen when higher radiotracer doses or patient factors (large body size, limited tolerance, or need for pharmacologic stress) could cause residual activity or artifacts if done same day. Spacing studies reduces overlap, lowers false results, and can increase patient comfort.

A stress test can indicate reduced blood flow to the heart during exertion, suggesting significant coronary artery disease, but it does not directly visualize or measure a specific arterial blockage. Abnormal stress test results point to ischemia and usually prompt further imaging (for example, invasive coronary angiography or CT coronary angiography) to confirm and locate blockages. Stress tests can give false positives or negatives.

A myocardial perfusion imaging (MPI) appointment usually lasts 2–4 hours for a single‑day protocol. Each scan acquisition takes about 20–40 minutes, with waits after radiotracer injection. A two‑day protocol spreads scans over two shorter visits (typically 1–2 hours each). Exercise or pharmacologic stress testing adds preparation, monitoring and recovery time (commonly another 30–60 minutes).
