A pyruvate blood test measures pyruvate, a key product of glucose metabolism, to help evaluate metabolic function. It’s used with lactate to investigate unexplained lactic acidosis, suspected pyruvate dehydrogenase or mitochondrial disorders, and to monitor treatment or metabolic control. Abnormal pyruvate-to-lactate ratios guide further genetic, enzymatic, or metabolic testing, especially in infants and people with unexplained neurological signs.

Normal serum pyruvate concentrations are typically low. Most laboratories report a reference range roughly 0.3–0.9 mg/dL (about 35–100 µmol/L), often measured on a fasting sample. Exact ranges vary by assay and lab, so values just outside this range should be interpreted alongside clinical findings and other metabolic tests. If results are abnormal, discuss them with your clinician.

High pyruvate levels arise when production exceeds utilization or mitochondrial conversion. Causes include increased glycolysis (heavy exercise, high‑carbohydrate intake, uncontrolled diabetes), impaired pyruvate oxidation or carboxylation (pyruvate dehydrogenase or pyruvate carboxylase deficiency, thiamine deficiency, mitochondrial disorders), systemic anaerobic states (hypoxia, sepsis, shock), liver disease, genetic enzyme defects, metabolic stress, or some drugs/toxins.

Pyruvate-related disorders include pyruvate dehydrogenase complex deficiency (causing lactic acidosis, developmental and neurological impairment), pyruvate carboxylase deficiency (lactic acidosis, hypoglycemia, neurodevelopmental delay), and pyruvate kinase deficiency (hemolytic anemia). Impaired pyruvate handling is also central to mitochondrial disorders, lactic acidosis syndromes, and cancer metabolic reprogramming (Warburg effect), where altered pyruvate flux supports tumor growth.

Low pyruvate levels cause impaired cellular energy production and can produce fatigue, muscle weakness and exercise intolerance; poor growth or weight gain in infants; developmental delay, hypotonia and seizures; breathing difficulty or rapid breathing; possible lactic acidosis or metabolic instability; and cognitive/learning problems. Symptoms vary by age and severity and often reflect underlying mitochondrial or metabolic disorders.

Pyruvate is a normal metabolic molecule essential for cellular energy, so it’s neither inherently “good” nor “bad.” As a supplement it has limited, inconsistent evidence for modest weight‑loss or performance benefits when taken in high doses. It’s generally safe in food amounts, but supplements can cause gastrointestinal upset and have uncertain effects in pregnancy, diabetes, or with medications—consult a clinician before use.
