Pan T - Cell / Early Myeloid Cell Marker : CD 7

CD7 is a transmembrane glycoprotein expressed mainly on T lymphocytes and natural killer (NK) cells. It serves as a pan–T‑cell marker used in flow cytometry and immunohistochemistry to identify T‑cell lineage, thymic precursors, and NK cells. Absent or reduced CD7 expression is diagnostically important in T‑cell malignancies, such as T‑cell acute lymphoblastic leukemia and certain cutaneous T‑cell lymphomas.

CD7 positive means a cell expresses the CD7 surface protein, a marker mainly found on T lymphocytes and natural killer cells. In blood or tissue testing (flow cytometry/immunophenotyping), CD7 positivity indicates T‑cell lineage. It’s a normal finding on healthy T cells, but patterns of expression—loss, dimming, or aberrant co‑expression—help clinicians detect or classify T‑cell disorders such as leukemias or lymphomas.

CD7 is a surface protein on T cells; its loss means T cells lack this usual marker. This can reflect abnormal T‑cell maturation or an acquired change seen in T‑cell leukemias/lymphomas (and sometimes in reactive conditions or infections). Loss of CD7 alone isn’t diagnostic — it prompts further evaluation with detailed immunophenotyping, molecular testing and clinical correlation to determine if a malignancy or benign process is present.

CD7 is a T‑cell surface antigen aberrantly expressed in a subset of acute myeloid leukemia (AML) cases—commonly about 20–30%. Its presence reflects lineage infidelity and is the most frequent lymphoid marker in AML. CD7 positivity can aid diagnosis and measurable-residual-disease monitoring and has been associated with distinct biology and generally worse treatment response and prognosis versus CD7‑negative AML.

Aberrant expression or loss of CD7 is primarily associated with T‑cell malignancies. Loss of CD7 is a common immunophenotypic feature in T‑cell acute lymphoblastic leukemia (T‑ALL), peripheral T‑cell lymphomas and cutaneous T‑cell lymphomas such as mycosis fungoides and Sézary syndrome. CD7 abnormalities may also be seen in some NK/T‑cell lymphomas and are used diagnostically to characterize T‑cell disorders.

Leukemia and lymphoma are usually identified by blood/marrow findings and specific markers: elevated lactate dehydrogenase (LDH) indicates high tumor burden. Immunophenotyping/flow cytometry detects lineage‑specific CD markers (e.g., CD19, CD10 for B‑cell; CD3 for T‑cell). Hodgkin lymphoma shows Reed–Sternberg cells positive for CD15/CD30. Molecular/cytogenetic markers (e.g., BCR‑ABL in CML) guide diagnosis and treatment.
