Two blood cultures are taken to improve detection of bloodstream infections and to distinguish true infection from skin contamination. Sensitivity falls if antibiotics were given before sampling, with low-level or intermittent bacteremia, or for fastidious organisms, and depends on collection technique and timing.

Paired cultures are two or more blood culture sets drawn from different sites and often at separate times to detect bloodstream infections and help distinguish true bacteremia from contamination. Typically two to three blood culture sets are drawn at the same time, each set comprising one aerobic and one anaerobic bottle from separate venipuncture sites. Adults usually provide 8–10 mL per bottle (about 20–30 mL per set). Two properly collected sets detect most bacteremias; more than three sets rarely increase diagnostic yield and are usually unnecessary.

Collect two complete blood culture sets by performing two separate venipunctures at different sites using strict aseptic technique. Clean each site with chlorhexidine/alcohol, draw recommended volume (usually 8–10 mL per bottle) into aerobic and anaerobic bottles for each set. Label with site and time, obtain both sets before starting antibiotics when possible, and transport promptly to the laboratory.
