Osmolality measures the concentration of dissolved particles (solutes) per kilogram of solvent—usually expressed in milliosmoles per kilogram (mOsm/kg). It reflects the body’s fluid balance and drives water movement between compartments. Clinically, plasma osmolality helps assess hydration and electrolyte status; normal serum osmolality is about 275–295 mOsm/kg. Abnormal values suggest dehydration, overhydration, or electrolyte disturbances.

Osmolality measures solute concentration per kilogram of solvent (osmoles/kg water), while osmolarity measures solute per liter of solution (osmoles/L). Osmolality is temperature- and pressure‑independent and is preferred for clinical assessment of body fluids; osmolarity can change with temperature and fluid volume. Both reflect osmotic activity but differ by reference (mass vs volume) and practical accuracy.

In dehydration, serum (plasma) osmolality is high because water loss concentrates solutes like sodium, raising plasma osmolality. Urine osmolality is usually high as the kidneys conserve water and produce concentrated urine. Exception: if the dehydration is due to inability to concentrate urine (e.g., diabetes insipidus), urine osmolality will be low despite dehydration.

Osmolarity is a simple measure of how many dissolved particles (salts, sugars, proteins) are in a liter of liquid. It shows how “concentrated” a fluid is and predicts water movement across cell membranes: water moves toward the side with higher osmolarity. Clinically, osmolarity helps assess body fluids, dehydration, and suitability of IV fluids. Units are usually milliosmoles per liter (mOsm/L).

When serum osmolality is low, body fluids become hypotonic and water shifts into cells, causing cellular swelling. In the brain this can lead to headache, nausea, confusion, lethargy, seizures and potentially coma from cerebral edema. Common causes include excess water intake, SIADH, heart or kidney failure. Electrolyte disturbances like hyponatremia, muscle cramps and weakness often accompany low osmolality.

Normal serum (plasma) osmolality is about 275–295 mOsm/kg, measured in milliosmoles per kilogram. Urine osmolality varies with hydration—typically roughly 50–1200 mOsm/kg—because kidneys concentrate or dilute urine. Values above or below the serum range suggest disturbances in fluid balance or electrolytes and usually warrant medical evaluation.
