Most pituitary MRI exams use intravenous gadolinium contrast with high-resolution, dynamic sequences because contrast greatly improves detection and characterization of microadenomas and other sellar lesions. Non-contrast MRI can be used when contrast is contraindicated (severe allergy, significant renal impairment, or early pregnancy), but sensitivity is lower. Discuss risks and alternatives with your clinician and radiologist.

A pituitary MRI with contrast typically takes about 20–45 minutes. The actual scan time in the magnet is usually 15–30 minutes; contrast injection and post-contrast sequences add several minutes. Allow extra time for preparation, positioning and paperwork, so the total appointment often runs 30–60 minutes. Individual times vary by facility, imaging protocols and patient factors.

A pituitary sella with contrast MRI is an MRI scan focused on the sella turcica to image the pituitary gland, performed after intravenous contrast (gadolinium). Contrast improves detection and characterization of micro- and macroadenomas, cysts, stalk lesions and cavernous sinus involvement. It helps evaluate hormonal or visual symptoms. The study is noninvasive; risks include rare contrast reactions and caution with severe kidney disease or pregnancy.

To visualize the pituitary on MRI, use a high-resolution dedicated pituitary protocol: thin-section (≤3 mm) T1-weighted images in sagittal and coronal planes, obtained before and after gadolinium contrast, plus dynamic contrast-enhanced sequences to detect microadenomas. Include T2-weighted images, a small field-of-view centered on the sella, and a head coil. 3T scanners improve detail; sedation may be used if needed.

Many pituitary tumors—particularly larger macroadenomas—can be seen on non‑contrast MRI, but small microadenomas (<10 mm) are often missed. Contrast-enhanced (especially dynamic) MRI markedly increases detection and delineation. CT without contrast is much less sensitive. If clinical suspicion is high or initial non‑contrast imaging is inconclusive, a contrast MRI is usually recommended.

Not always. Many brain MRIs are done without contrast and give excellent information (e.g., stroke or routine follow-up). Gadolinium contrast is added when better lesion characterization is needed—tumors, infection, inflammation, metastases, blood–brain barrier disruption, or some vascular studies. Contrast improves detection but has small risks (allergic reaction, gadolinium deposition, and caution with poor kidney function); decision depends on the clinical question.
