An MRI orbit with contrast is an imaging scan of the eye sockets performed after IV injection of a gadolinium-based contrast agent. Contrast improves visualization of orbital structures—optic nerves, extraocular muscles, fat and vessels—helping detect tumors, inflammation, infection, demyelination and vascular lesions. The test is noninvasive and quick; contrast is generally safe but warrants caution with severe kidney impairment or prior reactions.

An MRI of the orbits typically takes about 15–30 minutes. If contrast (gadolinium) is required or extra sequences are added, the scan and preparation can extend to 30–45 minutes, including IV insertion. Total appointment time varies with scanner type and whether brain imaging is included; allow extra time for arrival, paperwork, and positioning.

An MRI of the orbits is indicated for suspected orbital or optic nerve disease — sudden or unexplained vision loss, optic neuritis, proptosis, diplopia, orbital trauma, suspected orbital cellulitis/abscess, intra‑orbital mass or metastasis, thyroid eye disease, lacrimal gland disease, or pre/postoperative planning. It’s also used for persistent eyelid swelling or pain and when CT is non‑diagnostic or radiation should be avoided.

Yes—most centres allow clear fluids, including water, before an MRI with contrast. Drinking water is encouraged to help hydrate and aid contrast clearance; however many facilities ask you to avoid solid food for 4 to 6 hours and may give specific fasting or fluid limits if you'll have sedation or if you have kidney disease or diabetes. Always follow your imaging centre’s instructions and tell staff about medications or kidney problems.
