Cartilage mapping MRI is a quantitative MRI technique that measures biochemical properties of joint cartilage—collagen integrity, proteoglycan content and water—using sequences such as T2, T1ρ and dGEMRIC. It produces color maps and numeric values to detect early degeneration before structural damage appears, helping diagnosis, monitoring progression, and guiding treatment decisions for osteoarthritis and sports-related cartilage injuries without invasive procedures.

MRI is the preferred non‑invasive test to visualize knee cartilage and can detect tears, thinning, defects and degenerative changes. Specialized MRI sequences improve sensitivity and can map cartilage health. However, very small surface lesions or early biochemical cartilage damage may be missed; diagnostic arthroscopy remains the definitive method when imaging is inconclusive or clinical suspicion is high.

For routine morphologic cartilage imaging, proton-density–weighted sequences with fat suppression (PD‑FS) are preferred; high‑resolution 3D spoiled gradient‑echo (eg, 3D SPGR/3D GRE) sequences provide excellent surface detail and enable thin‑slice multiplanar reconstructions. For early biochemical changes, quantitative techniques—T2 mapping, T1ρ and dGEMRIC—are used. Choice depends on the clinical question: morphology versus biochemical assessment.

Costs vary widely by country and facility. In India a knee MRI typically runs about ₹2,000–₹10,000. In the UK it’s free on the NHS; privately expect roughly £200–£700. In the United States out‑of‑pocket prices range roughly $400–$3,000; insured patients usually pay a copay or deductible. Final cost depends on location, facility type, use of contrast, and insurance.

Detect cartilage damage by noting symptoms: joint pain worsened by movement, swelling, stiffness, reduced range of motion, crepitus, locking, or instability. Clinicians perform physical exams and use imaging—X‑rays can show joint space narrowing, MRI best visualizes cartilage defects, and ultrasound helps in some joints. Definitive diagnosis may require diagnostic arthroscopy; early assessment prevents progression.

T1 and T2 mapping are quantitative MRI techniques that measure, for each pixel, a tissue’s longitudinal (T1) and transverse (T2) relaxation times. These numeric maps objectively reflect tissue composition—helping detect fibrosis, inflammation, edema, iron or fat deposition—commonly used in cardiac and liver imaging. They enable noninvasive tissue characterization and comparison across scans or patients.
