An MR cisternogram is an MRI study using heavily T2-weighted or 3D sequences (for example, CISS or FIESTA) to sharply show cerebrospinal fluid–filled cisterns and subarachnoid spaces. It noninvasively visualizes CSF pathways, cranial nerves, and arachnoid cysts. When needed, small intrathecal contrast can be used to pinpoint cerebrospinal fluid leaks. It helps diagnose CSF leaks, nerve compression, and related conditions.

MRI cisternography begins with screening for contraindications and informed consent. For suspected CSF leak, intrathecal gadolinium via lumbar puncture may be given, followed by T1 fat-suppressed imaging. Monitor vitals and watch for complications.

A CT cisternogram usually takes about 1–3 hours total. The lumbar puncture and contrast injection take 15–30 minutes, while the CT images themselves are quick; technologists often wait 1–2 hours for contrast to circulate and may perform delayed scans. A cisternogram is performed in a nuclear medicine or radiology department. A nuclear medicine physician or radiologist oversees and interprets the study, while a trained nuclear medicine or radiology technologist injects the radiotracer and acquires images. The lumbar puncture is performed by a physician (often a radiologist or neurologist). The test is typically ordered by a neurologist or neurosurgeon.

To prepare for a cisternogram, tell your healthcare team about all medications (especially blood thinners), allergies, pregnancy or breastfeeding. You may be asked to stop certain drugs and follow fasting or fluid guidance. Arrange transport home and wear comfortable clothing.
