Gadolinium-based contrast agents (GBCA) are used for cardiac MRI. Injected intravenously (typically 0.1–0.2 mmol/kg), they enable first-pass perfusion imaging and late gadolinium enhancement to detect ischemia, infarction, scar, fibrosis, and myocarditis. GBCAs are generally safe but require caution in severe renal impairment and pregnancy; kidney function is usually checked beforehand.

Cardiac MRI with gadolinium contrast commonly causes mild reactions: nausea, headache, injection-site pain, taste changes or dizziness. Allergic reactions (itching, rash) can occur; rarely severe anaphylaxis. People with severe kidney disease risk nephrogenic systemic fibrosis. Other MRI-related issues include claustrophobia, loud noises and transient discomfort. Gadolinium can deposit in tissues; clinical significance remains uncertain.

A cardiac MRI with contrast typically takes about 30–60 minutes of scanning. Including IV placement, safety checks, breath‑holding instruction and image planning, the total appointment usually runs 45–90 minutes. Duration varies with the number of sequences, need for stress protocols, patient cooperation and heart rate—arrive early for paperwork and pre-scan preparation.

Before a cardiac MRI with contrast: arrange a recent kidney function test (eGFR within 3 months), notify staff about pregnancy, breastfeeding, allergies, implants or pacemakers. Fast for about 4 hours, wear metal‑free comfortable clothing and remove jewelry. Bring ID, referral and medication list. Continue usual meds unless told otherwise—if you take metformin or have reduced kidney function, ask whether to pause it. Arrange transport if sedated.

The safest MRI contrast agents are macrocyclic gadolinium‑based agents (e.g., gadoterate, gadobutrol, gadoteridol). Their tight molecular structure makes them least likely to release free gadolinium, reducing risk of nephrogenic systemic fibrosis. Use is minimized or avoided in severe kidney impairment, with non‑contrast options or alternatives preferred; contrast in pregnancy is reserved for essential cases only.

Cardiac contrast is a medical imaging agent injected intravenously to improve visualization of the heart and blood vessels during CT, MRI, or echocardiography. Iodine-based dyes are used for CT, gadolinium for MRI, and microbubble contrast for echo. It helps show anatomy, blood flow, and tissue damage.
