A brain MRI with perfusion is an imaging test that maps blood flow in brain tissue. It uses techniques such as contrast-based dynamic susceptibility or noncontrast arterial spin labeling to generate cerebral blood flow, volume, and transit-time maps. Perfusion MRI helps detect and characterize stroke, tumors, vascular lesions, and dementia-related changes, and assesses treatment response by revealing areas of reduced or increased perfusion.

Prepare the patient (consent, review renal function/allergies), place IV access, and choose technique: dynamic susceptibility contrast (DSC) with a rapid gadolinium bolus or non‑contrast arterial spin labeling (ASL). Acquire fast T2*‑weighted (DSC) or labeled/control sequences (ASL) during/after labeling, then post‑process to generate CBF, CBV, MTT and Tmax maps. Monitor vitals, watch for reactions, and interpret maps in clinical context.

Brain perfusion is the delivery of blood through cerebral vessels to brain tissue, supplying oxygen and glucose and removing carbon dioxide and metabolic waste. Adequate perfusion maintains neuronal function; impairment from ischemia, stroke, or low blood pressure can cause tissue injury. Clinically it’s assessed using imaging (CT/MR perfusion, Doppler) and by measuring cerebral blood flow and perfusion pressure.

A brain perfusion test typically takes between 20 and 60 minutes. CT perfusion scans are quick — about 10 to 20 minutes — while MR perfusion studies usually require 30 to 60 minutes. Allow extra time for registration, intravenous contrast injection, brief monitoring, and image processing; the full appointment commonly lasts 30 to 90 minutes depending on the facility.

Perfusion scans are generally safe but carry small risks: exposure to low-level ionizing radiation, rare allergic reactions to the tracer, and minor injection-site pain or bruising. If a ventilation component is used, transient coughing or bronchospasm can occur in people with reactive lung disease. Pregnant or breastfeeding patients need special consideration. Rarely, inaccurate results lead to further tests or treatment.

A stress perfusion MRI usually requires about 30–60 minutes of imaging; the full appointment — including preparation, ECG hookup, administration of a vasodilator or pharmacologic stress agent, contrast injection, monitoring, and brief recovery — commonly takes 45–90 minutes. Times vary with center protocols, patient cooperation and whether additional cardiac sequences or repeat images are needed.
