MRI (including MR angiography) can clearly visualize the aorta, showing its size, wall structure, blood flow and detecting aneurysms, dissections, inflammation or narrowing. It’s non‑ionizing, useful for thoracic and abdominal aortic assessment and follow‑up. Contrast-enhanced or non‑contrast techniques are available. Limitations include availability, scan time, and contraindications such as certain implants or severe claustrophobia.

MRI angiography can detect significant blockages in some coronary and larger heart vessels noninvasively, and cardiac MRI also assesses heart muscle damage and ischemia. However, it has lower spatial resolution and is less reliable for small or heavily calcified coronary arteries. CT coronary angiography and invasive coronary angiography remain more accurate for diagnosing coronary artery blockages. Contrast use, heart motion, and patient factors can limit MRA.

Symptoms of aorta problems can include sudden, severe chest or upper‑back pain described as a ripping/tearing sensation (aortic dissection), intense abdominal or back pain with a pulsatile abdominal mass (abdominal aortic aneurysm), shortness of breath, fainting or dizziness, unequal pulses or blood pressures in the arms, hoarseness or difficulty swallowing from compression, palpitations, and signs of shock if rupture occurs.

Doctors check the aorta by taking a history and physical exam (palpating the abdomen, checking pulses, listening for bruits) and using imaging: abdominal ultrasound for aneurysm screening; transthoracic or transesophageal echocardiography, CT angiography or MR angiography to evaluate the thoracic aorta, dissections or aneurysms. Blood pressure control and serial imaging monitor known disease; urgent CT/echo is used if dissection is suspected.
