MRI of the perianal region is a high-resolution pelvic MRI that visualizes the anal canal, internal and external sphincters, intersphincteric plane and surrounding soft tissues. Acquire high‑resolution axial‑oblique (perpendicular) and coronal‑oblique (parallel) sequences: T2‑weighted ± fat suppression (STIR/T2FS), T1‑weighted pre‑contrast, and T1 fat‑sat post‑contrast in axial and coronal planes. Add diffusion if needed, cover anal canal to perineum, and mark external openings.
