MRI Heel

An MRI for heel pain shows detailed soft-tissue and bone changes: plantar fascia thickening, degeneration or partial tear; Achilles tendonitis, tendinosis or rupture; retrocalcaneal or subcutaneous bursitis; nerve entrapment; bone marrow edema or stress fracture; calcaneal bone lesions or cysts. It helps localize inflammation, fluid collections and structural damage to guide treatment and rule out other causes requiring surgery.

For heel pain, initial plain X‑ray rules out fracture or calcaneal spur. For soft‑tissue causes like plantar fasciitis, ultrasound is excellent to assess fascia thickness and inflammation. MRI is the most sensitive test for complex cases, deep soft‑tissue injury or suspected bone marrow edema and is used when diagnosis is unclear or symptoms persist despite treatment. Choice depends on suspected cause.

An MRI of the foot visualizes bones, joints, cartilage, tendons, ligaments and soft tissues. It can detect fractures and bone marrow edema, stress fractures, tendon tears or tendinopathy (e.g., Achilles, plantar fascia), ligament sprains, joint effusions, cartilage or osteochondral lesions, Morton’s neuroma, cysts, infections, inflammation, arthritis and tumors. MRI helps localize injury, assess severity, and guide treatment planning.

Yes — you can usually walk immediately after a foot MRI. MRI is noninvasive and requires no recovery time. Exceptions: if you had an injection, biopsy, sedation, or local anaesthetic during the visit, follow your clinician’s post‑procedure instructions (they may advise rest or limited weightbearing). If you have dressings or swelling, wear supportive shoes and avoid strenuous activity until advised otherwise.

MRI can detect structural causes of nerve injury in the foot — for example compression, masses, inflammation, or nerve swelling — and specialized MR neurography can show nerve signal changes. However, routine MRI is less sensitive for microscopic or metabolic neuropathies; nerve conduction studies and EMG better assess functional nerve damage. Your doctor will choose imaging or electrodiagnostics based on symptoms and suspected cause.

For heel pain, clinicians start with a clinical examination: history, palpation of the plantar fascia, and the Windlass test (pain on dorsiflexion of the toes). Imaging may follow: X-ray to exclude fractures or calcaneal spurs, ultrasound to assess plantar fascia thickness, and MRI for persistent or complex cases to evaluate tears or stress fractures.
