MR Venogram

An MR venogram is used to noninvasively visualize veins—most often the cerebral venous sinuses—to detect blood clots (cerebral venous sinus thrombosis), venous stenosis or malformations, and causes of raised intracranial pressure. It assists in evaluating unexplained headaches or stroke‑like symptoms, aids preoperative planning, and guides endovascular procedures, providing venous mapping without ionizing radiation or invasive catheter angiography.

MRI (magnetic resonance imaging) produces detailed cross-sectional images of organs, bones, soft tissues and pathology using magnetic fields and radio waves. MRV (magnetic resonance venography) is a specialized MRI that maps venous structures and blood flow, often using contrast or time-of-flight techniques to detect venous thrombosis, stenosis or malformations. In short: MRI images anatomy broadly; MRV specifically visualizes veins.

An MR venogram scan itself usually takes about 30–60 minutes. If contrast is used, additional imaging sequences may add time. Allow extra minutes for registration, IV insertion, positioning, and post-scan checks — the overall appointment often runs 60–90 minutes. Times vary with scanner type, patient needs (claustrophobia, mobility) and facility scheduling. Ask your imaging center for exact timing.

An MR venogram can be done with or without contrast. Non‑contrast techniques (time‑of‑flight or phase‑contrast) are possible, but contrast‑enhanced MRV using gadolinium usually provides clearer vascular detail and is preferred for suspected thrombosis or complex anatomy. Choice depends on clinical indication, patient factors and local protocol; gadolinium is used cautiously in severe renal impairment and some pregnancy cases.

MR venography is generally highly accurate for detecting central venous problems—contrast‑enhanced MRV typically shows sensitivity and specificity above 90% for major venous thrombosis. Accuracy is lower with non‑contrast/time‑of‑flight techniques, in small peripheral veins, or with slow flow, metal implants or motion artifacts. Equivocal studies are often confirmed with ultrasound, CT venography, or conventional venography.

Magnetic resonance venography (MRV) images are reviewed by a radiologist; a preliminary read is often available the same day, especially for urgent cases. Final reports are usually issued within 24 to 48 hours, though non‑urgent outpatient results can take up to a week depending on the facility and workload. Your referring clinician will communicate findings and next steps once the report is released.
