Multiparametric MRI of the prostate combines high resolution T2-weighted anatomical imaging with diffusion-weighted imaging (DWI) including ADC maps and dynamic contrast-enhanced (DCE) sequences. It helps detect, localize and stage clinically significant prostate cancer, guide targeted biopsy and active surveillance, reduce unnecessary biopsies, and provides a non‑ionizing, detailed assessment for treatment planning.

The prostate is a single, walnut-sized gland surrounding the urethra below the bladder that produces prostatic fluid—enzymes and slightly acidic secretions important for sperm activation; it’s prone to BPH, prostatitis and cancer. The seminal vesicles are paired, sac-like glands behind the bladder that secrete alkaline, fructose-rich fluid supplying about 60–70% of semen volume and energy for sperm.

If prostate cancer spreads to the seminal vesicles it is considered locally advanced (stage T3b). This raises the risk of recurrence and often requires more aggressive treatment—combinations of surgery, radiation and androgen‑deprivation (hormone) therapy. It may cause pelvic pain, blood in semen or worsening urinary symptoms. Overall prognosis is less favorable than organ‑confined disease, so close monitoring and multimodal therapy are usually recommended.

A prostate multiparametric (mpMRI) scan usually requires about 20–45 minutes of actual scanning. Allow around 30–60 minutes for the entire appointment to include check-in, positioning, and contrast injection/recovery if used. Duration can vary with scanner type, use of an endorectal coil, added sequences, and patient movement; complex exams may take up to about 60–75 minutes.

Multiparametric MRI (mpMRI) is less invasive and often better at detecting clinically significant prostate cancers and guiding targeted biopsies, reducing unnecessary systematic biopsies. However, mpMRI cannot fully replace biopsy: tissue sampling remains the gold standard for definitive diagnosis and grading, because mpMRI can miss some cancers and results depend on scanner quality and reader experience.

Costs vary widely by country, facility, and whether contrast or fusion biopsy is included. In the UK an mpMRI is usually free on the NHS; private scans cost roughly £300–£900. In India expect about ₹4,000–15,000. In the US typical charges range from about $500 to $3,000–4,000 before insurance; Medicare or private insurance may cover part or all depending on indication.
