Malarial Parasite species identification

Malaria species are identified primarily by microscopy of thick and thin blood films—thick for parasite detection and thin for species morphology and parasitaemia. Rapid diagnostic tests detect species-specific antigens (eg HRP2 for P. falciparum) but have limits. PCR offers highest sensitivity and definitive speciation, including mixed infections. Clinical history (travel), repeat testing for low parasitaemia, and expert review support accurate identification.

The four human malaria parasite species are Plasmodium falciparum, Plasmodium vivax, Plasmodium ovale, and Plasmodium malariae. These single-celled parasites are transmitted by Anopheles mosquitoes and cause varying illness severity; P. falciparum most often leads to severe disease, while P. vivax and P. ovale can form dormant liver stages. P. malariae typically causes milder, chronic infections.

The seven types of malaria (human-infecting Plasmodium species) are Plasmodium falciparum, Plasmodium vivax, Plasmodium ovale curtisi, Plasmodium ovale wallikeri, Plasmodium malariae, Plasmodium knowlesi, and Plasmodium simium. Falciparum causes the most severe disease; vivax and ovale species can form dormant liver stages that relapse, while knowlesi and simium are primarily zoonotic (transmitted from animals).

Malaria is diagnosed primarily by microscopic examination of thick and thin blood smears to detect and identify Plasmodium species; rapid diagnostic tests (RDTs) that detect parasite antigens (e.g., HRP2, pLDH) provide quick bedside results. PCR is used for species confirmation or low-level infections. Serology isn't useful for acute diagnosis. Prompt testing with microscopy or RDTs guides treatment.

Identify Anopheles by adults’ features: long palps equal to the proboscis, characteristic dark-and-light spotted wings, and a habit of resting with the body angled upward (~45°). Eggs are laid singly on water (not in rafts). Larvae lie parallel to the water surface and lack a distinct breathing siphon. These combined characters distinguish Anopheles from other mosquitoes.

Prepare thick and thin blood films, stain with Giemsa, and examine under oil immersion (100x). Use the thick film to detect parasites and the thin film to identify species. Look for ring forms, trophozoites, schizonts and gametocytes: P. falciparum shows multiple rings per RBC and crescent-shaped gametocytes; P. vivax/ovale enlarge RBCs with Schüffner’s dots; P. malariae has band forms. Estimate parasitemia by counting parasites.
