Immunoglobulin M (IgM)

Immunoglobulin M (IgM) is the body's first antibody produced during a new infection. It circulates mainly in blood and lymph as a large pentamer, enabling strong pathogen agglutination and efficient activation of the complement system. IgM typically indicates recent or acute infection because it appears early with lower affinity, before class-switching to other antibodies like IgG, and is measured in serology tests.

Low IgM means reduced levels of the first-line antibody (IgM), which can reflect a primary immunodeficiency (selective IgM deficiency or common variable immunodeficiency), bone marrow disease, protein loss (e.g., nephrotic syndrome), certain drugs, or age-related change. It increases risk of recurrent infections and poor vaccine responses. Doctors usually repeat immunoglobulin tests, review infection history, and may refer to an immunologist.

A positive IgM usually indicates a recent or early immune response to infection (or recent vaccination). It can appear before IgG and may be followed by seroconversion. False positives or cross‑reactivity are possible, so interpret results alongside symptoms, exposure history and confirmatory tests (repeat serology, PCR). Consult your healthcare provider for follow‑up, further testing and treatment decisions.

Markedly elevated IgM is classically caused by Waldenström macroglobulinemia (lymphoplasmacytic lymphoma) and IgM‑type monoclonal gammopathy of undetermined significance (MGUS). Other B‑cell malignancies can raise IgM, including marginal zone lymphoma, chronic lymphocytic leukemia/small lymphocytic lymphoma, and—rarely—IgM myeloma. Symptoms and treatment vary depending on the cancer type and IgM‑related complications such as hyperviscosity.

IgM positivity suggests a recent or current dengue infection but is not definitive on its own. IgM typically appears around day 4 to 5 after symptoms, can persist for weeks to months, and may cross-react with other flaviviruses or give false positives. Diagnosis is confirmed by NS1 antigen, PCR, paired sera or clinical assessment. Discuss results with a clinician for appropriate follow-up and testing.

IgM positivity indicates a recent or current infection, not a condition to "cure." Management depends on the specific pathogen: confirm diagnosis with clinical assessment and targeted tests (repeat antibody or PCR), then follow pathogen‑directed therapy (antibiotics, antivirals, antiparasitics as indicated). Provide supportive care, rest, hydration, isolate if contagious, and arrange follow‑up testing with your clinician. Do not self‑treat.
