An HTLV‑1/2 test detects infection with human T‑lymphotropic viruses 1 and 2 by checking for antibodies or viral genetic material. It’s used to screen blood and tissue donors, diagnose infections in people with symptoms (e.g., adult T‑cell leukaemia/lymphoma or HTLV‑associated myelopathy), guide counselling on transmission risks (blood, sex, breastfeeding), and monitor or confirm infection for clinical management.

HTLV (types 1 and 2) is primarily a blood-borne virus but can be sexually transmitted. It isn’t typically labeled solely as a sexually transmitted disease, yet unprotected sexual contact is a recognized route of spread alongside blood exposure, needle sharing, and mother-to-child transmission (especially breastfeeding). Prevention includes safer sex, screened blood products, avoiding needle sharing, and counseling infected mothers.

Human T-lymphotropic virus type 2 (HTLV-2) is less pathogenic than HTLV-1 but has been linked mainly to neurological disease — a chronic, progressive myelopathy resembling HAM/TSP (HTLV-associated myelopathy/tropical spastic paraparesis). HTLV-2 is also occasionally associated with peripheral neuropathy and increased susceptibility to infections, but it is far less commonly connected to leukemia than HTLV-1.

A positive HTLV test means you have evidence of HTLV infection (antibodies or viral material). It shows past or current infection but doesn’t mean you have disease—most carriers remain asymptomatic. Confirmatory testing and clinical follow‑up are needed. HTLV‑1 can rarely cause adult T‑cell leukemia/lymphoma or neurologic disease; avoid blood/tissue donation and seek specialist advice about transmission and monitoring.

HTLV infection is usually lifelong but most people remain asymptomatic. A minority develop serious diseases: HTLV‑1 carriers have about a 2–5% lifetime risk of adult T‑cell leukemia/lymphoma and roughly 0.25–4% risk of HTLV‑associated myelopathy/tropical spastic paraparesis. These conditions can be progressive with significant disability and mortality. Transmission prevention, screening in high‑risk groups, and medical follow‑up are important.

Most people infected with HTLV remain asymptomatic; when symptoms occur they usually start years to decades after infection. About 2–5% develop adult T‑cell leukemia/lymphoma, often 20 to 30 years later, and roughly 0.25–3% develop HTLV‑associated myelopathy/tropical spastic paraparesis, typically years to decades after exposure. Rarely, a brief flu‑like illness may occur soon after infection.
