High-resolution CT of the chest detects many lung diseases, especially parenchymal and small‑airway disorders. Commonly identified conditions include interstitial lung diseases (idiopathic pulmonary fibrosis, nonspecific or usual interstitial pneumonia), hypersensitivity pneumonitis, sarcoidosis, bronchiectasis, emphysema/COPD, small‑airways disease, atypical infections and scarring; it also helps characterize lung nodules and masses, and for staging and monitoring disease progression.

High-resolution CT (HRCT) side effects include exposure to ionizing radiation with a small long-term increased cancer risk. If iodinated contrast is used, possible allergic reactions, a warm flushing sensation, nausea, and rare contrast‑induced kidney injury can occur. Some people experience anxiety or mild discomfort during the scan. Serious immediate reactions are uncommon but possible.

You often do not need IV contrast for a routine lung CT that assesses airways and parenchyma (e.g., nodule detection, interstitial lung disease). Contrast is required for CT pulmonary angiography to diagnose pulmonary embolism and is helpful for cancer staging, mediastinal evaluation, infections/abscesses, or vascular lesions.
