A good hepatitis C viral load is undetectable — the treatment goal is sustained virologic response (SVR12), meaning HCV RNA is below the test’s detection limit 12 weeks after finishing therapy, which indicates cure. Historically, lower pre-treatment loads (<800,000 IU/mL) predicted better response to interferon-era therapy, but with modern direct-acting antivirals most patients achieve undetectable levels.

High hepatitis C viral load often does not produce distinct symptoms and many people are asymptomatic. When symptoms occur (acute or chronic infection) they include fatigue, fever, nausea, loss of appetite, abdominal pain, dark urine, pale stools, jaundice, and muscle or joint aches. Advanced disease signs—cirrhosis or liver failure—include swelling, easy bruising, confusion, and severe jaundice.

Check HCV RNA at diagnosis to confirm active infection and obtain a baseline viral load before treatment. During therapy measure viral load as recommended (often at week 4 for early response and at end of treatment). Most importantly, test 12 weeks after completing antiviral therapy (SVR12) to confirm cure. In suspected acute infection, repeat testing at 6–12 weeks documents spontaneous clearance.

A positive hepatitis C antibody test means you were exposed to HCV; a viral RNA test is needed to confirm active infection. If RNA positive, clinicians evaluate liver function, viral load and fibrosis and recommend direct-acting antiviral treatment, which cures most people. You’ll also get counseling on transmission prevention, vaccinations (A and B), alcohol avoidance and regular liver monitoring.

A "bad" viral load means a high or persistently detectable amount of virus in the blood, signaling active replication, greater risk of disease progression, and higher chance of transmission. For HIV, "undetectable" is generally <50 copies/mL; persistent levels above about 200 copies/mL often indicate treatment failure. High viral load usually prompts closer monitoring and treatment adjustment.

HCV quantitative viral load is reported in IU/mL. The lower limit of quantification for common assays is around 10–15 IU/mL (values below are “undetectable”); the measurable range typically extends up to ~100,000,000 IU/mL. Active chronic infections commonly show 10^3–10^7 IU/mL. Clinically, a “normal” result is undetectable; any detectable HCV RNA indicates infection and needs follow-up.
