CAP stands for Controlled Attenuation Parameter. It’s a noninvasive FibroScan measure of ultrasound attenuation (reported in dB/m) that estimates liver fat (steatosis). Higher CAP values indicate greater hepatic fat content; results are interpreted using cutoffs for none, mild, moderate and severe steatosis and should be considered alongside clinical findings and liver stiffness.

A normal FibroScan shows low liver stiffness and minimal fat. Liver stiffness (kPa) is generally considered normal below about 5.5–7.0 kPa; values under ~7 kPa usually indicate no significant fibrosis. The controlled attenuation parameter (CAP) for steatosis is typically normal below ~238 dB/m (no steatosis). Interpretation depends on clinical context; discuss borderline or higher values with your clinician.

A CAP (controlled attenuation parameter) below about 238 dB/m is generally considered indicative of little or no liver fat and thus a "good" result. Values roughly 240–260 dB/m suggest mild steatosis, 260–290 moderate, and >290 dB/m marked steatosis. CAP should be interpreted alongside clinical context and measurement quality by your clinician.

FibroScan costs vary widely by country and setting. On public health systems it’s usually free; private clinics typically charge roughly £100–£350 in the UK, INR 2,000–7,000 in India, and about $200–$1,000 in the US. Insurance often covers it when clinically indicated. Confirm local clinic pricing and insurance/billing details before booking.

To lower a FibroScan CAP score, lose weight (5–10% body weight), eat a calorie-controlled Mediterranean-style diet low in refined carbs, added sugars and saturated fats, boost regular aerobic and resistance exercise, limit or avoid alcohol, and control diabetes and lipids. Treat any viral hepatitis and follow your healthcare team’s plan with repeat imaging to monitor improvement.

Grade 3 fatty liver is severe hepatic steatosis in which fat accumulates in more than two-thirds (>66%) of liver cells, often seen as a very bright, enlarged liver on ultrasound with loss of normal vascular detail. It carries higher risk of inflammation, fibrosis and progression to cirrhosis. Management focuses on weight loss, blood sugar and lipid control, avoiding alcohol, and specialist evaluation.
