Extended APLA Test

An APLA test in pregnancy checks for antiphospholipid antibodies—lupus anticoagulant, anticardiolipin, and anti–β2 glycoprotein I. These antibodies raise the risk of blood clots, recurrent miscarriage, stillbirth, preeclampsia and fetal growth restriction. Diagnosis usually requires persistently positive results at least 12 weeks apart. If positive, treatments like low‑dose aspirin and heparin can lower complication risk.

APLA testing includes: lupus anticoagulant assays (functional coagulation tests such as dilute Russell’s viper venom time with mixing/confirmatory steps), anticardiolipin antibodies (IgG and IgM), and anti–β2‑glycoprotein I antibodies (IgG and IgM). For diagnosis, positive results must be persistent on repeat testing at least 12 weeks apart to exclude transient antibodies.
