Endomysial IgG Antibody

Endomysial antibody IgG is an autoantibody of the IgG class that targets tissue transglutaminase in the muscle endomysium. It helps detect celiac disease, especially in people with IgA deficiency when IgA-based tests are unreliable. A positive EMA‑IgG is highly specific for celiac disease, but diagnosis is confirmed by clinical assessment, serology and intestinal biopsy or response to a gluten-free diet.

Anti-endomysial antibodies (EMA) are IgA autoantibodies highly specific for celiac disease. They are typically positive in active gluten-sensitive enteropathy and its skin manifestation, dermatitis herpetiformis, and correlate with small-intestinal villous atrophy. EMA usually become negative on a strict gluten-free diet and can be falsely negative in IgA deficiency; they are used for diagnosis and monitoring of celiac disease.

The IgG test for celiac disease measures IgG antibodies against gliadin or deamidated gliadin peptides. It’s used when IgA deficiency or young age makes IgA tests unreliable. A positive IgG supports possible celiac disease but is less specific than IgA anti‑tissue transglutaminase testing; diagnosis usually requires confirmatory evaluation (duodenal biopsy or HLA testing). Stay on a gluten-containing diet before testing.

Cost varies by country and setting: often free on public health systems; privately it typically ranges from about $30–150 (US), £20–80 (UK), or ₹1,200–3,500 (India). Factors include the lab, whether it’s part of a panel, and insurance coverage. Check with local labs or your clinician for exact pricing and whether repeat or reflex testing is needed.

Celiac disease has a strong genetic component. Most affected people carry HLA‑DQ2 or HLA‑DQ8 genes, but those genes alone aren’t sufficient—many carriers never develop disease. Other non‑HLA genes also contribute, and environmental exposure to gluten triggers it. First‑degree relatives have an elevated risk (roughly 10%), so family screening is often recommended.

A negative endomysial antibody (EMA) test means no detectable IgA antibodies against endomysium, which makes active celiac disease less likely—especially if total IgA is normal and you are eating gluten. It is not definitive: false negatives can occur with IgA deficiency, low gluten intake, or early disease. If symptoms or clinical suspicion remain, further testing (tTG IgA, total IgA, or small‑bowel biopsy) is advised.
