An NCV upper limb test (nerve conduction velocity) measures how fast electrical impulses travel along the arm’s nerves to detect damage, compression, or slowed conduction. Surface electrodes stimulate and record responses; mild electrical pulses are felt. It’s used for carpal tunnel, peripheral neuropathy, and radiculopathy diagnosis. Tests typically take 20–60 minutes, may cause brief discomfort, and usually need no special preparation.

EMG (electromyography) records a muscle’s electrical activity at rest and during contraction to detect muscle disorders and denervation. NCV (nerve conduction velocity) measures how fast and how strongly electrical signals travel along peripheral nerves to identify nerve compression, demyelination, or axonal loss. Together they help distinguish primary muscle disease from peripheral nerve or neuromuscular junction problems.

Costs vary by country and facility. In the US an EMG/NCV typically ranges roughly $200–$2,000 self-pay (clinic tests at the low end; hospital settings and extensive studies at the high end); insurance or Medicare often covers most costs. In India fees commonly run about ₹800–₹6,000. In the UK the test is usually provided free through the public health system. Final price depends on location, facility and extent of testing.

An EMG/NCV can cause discomfort: needle EMG produces sharp pricks, brief aching and muscle twitching during insertion and contraction, while nerve conduction testing uses small electrical shocks that feel like tingling or brief stings. Pain varies by person and site, but most find it tolerable and short‑lived. Clinicians use techniques to minimize discomfort, and complications are rare.

A positive EMG indicates abnormal electrical activity in muscles, showing signs of denervation, reinnervation or spontaneous activity (fibrillations, fasciculations). It suggests a nerve or muscle disorder—such as neuropathy, radiculopathy, motor neuron disease, or myopathy—and helps localize and stage the lesion. Results often prompt further tests (nerve conduction studies, imaging, blood tests) and guide treatment and specialist referral.

Nerve damage can sometimes improve but cure depends on type and severity. Peripheral nerves often regenerate slowly (millimetres per day) and may recover with time, physiotherapy, medications or surgery; severe injuries or transections may need grafting and might not fully restore function. Damage to the brain or spinal cord is much less likely to fully recover. Prognosis varies—early assessment and treatment improve outcomes.
