Tissue Doppler imaging (TDI) is an echocardiography technique that measures the velocity of myocardial (heart muscle) motion rather than blood flow. It quantifies systolic and diastolic velocities (s', e', a') at specific sites (for example, the mitral annulus) to assess global and regional ventricular function, detect diastolic dysfunction, and help estimate filling pressures alongside conventional echo.

An echo Doppler assesses heart structure, chamber size and pumping function and evaluates heart valves. It uses ultrasound to visualize cardiac motion and Doppler to measure blood flow speed and direction, helping detect valve disease, heart failure, congenital defects, clots or reduced blood supply. It also examines blood flow in peripheral arteries and veins to identify blockages, narrowing or venous reflux and to monitor treatment.

Tissue Doppler imaging noninvasively quantifies myocardial velocities, allowing objective assessment of systolic (s') and diastolic function (e', E/e' ratio) and estimation of filling pressures. It detects regional wall motion abnormalities and early dysfunction (ischemia, cardiotoxicity), identifies mechanical dyssynchrony for CRT planning, offers reproducible timing and velocity measurements, and can be performed at the bedside with standard echocardiography equipment.

Normal mitral annular tissue Doppler (TDI) velocities: early diastolic (e') ≈10–15 cm/s lateral, ≈7–10 cm/s septal; systolic (s') ≈8–12 cm/s lateral, ≈6–8 cm/s septal. e' falls with age. An E/e' ratio <8 usually indicates normal LV filling pressure, 8–15 is indeterminate, and >15 suggests elevated filling pressures. Values vary with technique and patient factors.

Doppler echocardiography assesses heart structure, valve function and blood flow and can reveal indirect signs of coronary artery blockage—such as regional wall‑motion abnormalities, reduced ejection fraction or ischemia on stress echo. It cannot directly visualize coronary artery blockages; definitive detection requires coronary angiography (invasive) or CT coronary angiography, and other tests like stress testing.

Use tissue Doppler imaging (TDI) during echocardiography by selecting TDI mode, obtaining apical views, and aligning the Doppler cursor parallel to myocardial motion. Sample at the mitral annulus and ventricular walls to record S', e' and a' velocities. Optimize settings (low wall filter, appropriate sweep speed, gain) and average septal and lateral e' for diastolic assessment (E/e').
