An obstetric Doppler is a handheld ultrasound device that uses the Doppler effect to detect and measure fetal heart rate and blood flow in fetal and uteroplacental vessels. It provides non‑invasive, real‑time audio and numeric monitoring during prenatal visits to assess fetal wellbeing and placental circulation. It’s safe for routine checks but not a substitute for detailed ultrasound scans or diagnostic fetal assessments.

A Doppler scan in pregnancy is a non‑invasive ultrasound that uses the Doppler effect to measure blood flow in the placenta, umbilical cord and fetal vessels. It checks uterine, umbilical and fetal cerebral circulation to assess placental function and detect issues like growth restriction, fetal distress or preeclampsia. It is safe, painless and commonly used in higher‑risk pregnancies to guide monitoring and management.

A normal pregnancy Doppler shows a positive end‑diastolic waveform in the umbilical artery (no absent or reversed end‑diastolic flow). Systolic/diastolic (S/D) ratio is generally <3 in late pregnancy; pulsatility index (PI) and resistive index (RI) should fall within gestation‑specific reference ranges. Interpretation depends on gestational age and clinical context and should be done by the treating clinician.

Doppler devices detect the fetal heartbeat and can pick it up from about ten to twelve weeks, but they cannot reliably determine the baby’s sex. Heart rate patterns or the “over 140 bpm equals girl” rule are not accurate predictors. Fetal sex is determined by viewing genital anatomy on ultrasound (usually around 18–20 weeks) or by genetic tests like cell-free DNA.

A positive Doppler indicates abnormal blood flow—such as occlusion, deep vein thrombosis, significant arterial narrowing, or fetal blood‑flow compromise. It triggers further evaluation (repeat ultrasound or CT/MR angiography), urgent treatment (anticoagulation, thrombolysis, or surgery) and specialist referral to prevent complications like pulmonary embolism, limb ischemia, or fetal distress. Follow‑up monitoring and risk‑factor management are required.
