An aortic Doppler is a noninvasive ultrasound test that uses Doppler sound waves to evaluate blood flow and velocity in the aorta and across the aortic valve. It helps detect narrowing (stenosis), regurgitation, aneurysm-related flow changes, and dissection, and estimates pressure gradients and cardiac output. It's commonly performed during echocardiography or vascular ultrasound to guide diagnosis and treatment.

A normal aortic valve Doppler typically shows a peak systolic velocity under about 2.0 m/s, a mean transvalvular gradient generally below 20 mmHg (often <10 mmHg), and an aortic valve area roughly greater than 1.5–2.0 cm². Higher velocities, gradients, or a smaller valve area suggest progressive aortic stenosis and warrant cardiology assessment and follow-up imaging.

An aortic ultrasound visualizes the abdominal or thoracic aorta to detect and measure aneurysms, assess wall abnormalities (dissection, plaque) and evaluate blood flow. It’s used for screening high‑risk individuals, monitoring aneurysm size over time, guiding treatment decisions, and investigating symptoms such as unexplained abdominal or back pain. It’s noninvasive, quick, and helps assess rupture or complication risk.

Normal Doppler peak systolic velocity in the abdominal aorta is approximately 40–100 cm/s (0.4–1.0 m/s). Flow is expected to be laminar and often triphasic; markedly higher velocities or turbulence may indicate stenosis, aneurysm, or other pathology. Exact cutoffs vary by lab and segment, so interpret alongside anatomical imaging and clinical context. For renal artery assessment, different thresholds apply.

A positive Doppler ultrasound means abnormal blood flow was detected—examples include a blood clot (DVT), arterial narrowing or poor perfusion, or venous insufficiency. Management depends on the cause: further imaging and specialist referral, initiation of anticoagulation for clots, or interventions (angioplasty, stenting, or surgery) for significant arterial disease, plus monitoring and risk‑factor control.

Doppler ultrasound measures blood flow and can detect blockages in peripheral and carotid arteries. For coronary (heart) artery blockages, standard Doppler generally cannot directly visualize them; echocardiographic Doppler may show indirect signs (abnormal wall motion or reduced flow), especially during stress testing. Definitive diagnosis of coronary blockages requires CT coronary angiography or invasive coronary angiography.
