D-Dimer

A high D‑dimer means there’s increased clot formation and breakdown in the body. It’s sensitive but nonspecific: elevated in conditions like deep vein thrombosis, pulmonary embolism, disseminated intravascular coagulation, recent surgery, trauma, infection, inflammation, or pregnancy. A raised result prompts clinical correlation and usually further testing (e.g., ultrasound or CT angiography) to confirm or exclude a clot and identify the cause.

D-dimer is a blood test marker of fibrin breakdown; markedly elevated levels suggest active clot formation and breakdown, most commonly venous thromboembolism — deep vein thrombosis and pulmonary embolism. It’s also raised in disseminated intravascular coagulation, recent surgery, trauma, infection, inflammation, cancer and pregnancy, so clinical context is essential when interpreting results.

High D-dimer isn't treated directly; care targets the underlying cause. If the rise indicates a clot (DVT/PE), anticoagulation is started. If related to infection, inflammation, surgery, pregnancy, or cancer, those conditions are managed and levels monitored. Management may include risk‑factor modification and follow‑up testing; decisions depend on clinical context and diagnostic imaging.

Many cancers — especially advanced or metastatic tumors — raise D‑dimer. The most commonly associated malignancies include pancreatic, lung, colorectal/gastrointestinal, ovarian and gastric cancers, primary brain tumors, and hematologic cancers (leukemia, lymphoma). Elevated D‑dimer often reflects tumor‑related coagulation activation, venous thromboembolism, or disseminated intravascular coagulation, and tends to correlate with tumor burden and prognosis.

Psychological stress alone usually does not directly raise D‑dimer. However, severe physical stress or systemic inflammation — and conditions sometimes triggered or worsened by stress (infection, trauma, surgery, dehydration, hypercoagulable states) — can elevate D‑dimer. D‑dimer is a nonspecific marker of clot breakdown; an elevated result needs clinical correlation, further evaluation, and possibly imaging to rule out thrombosis.

Certain medications can raise D-dimer by promoting clot formation or increasing fibrin breakdown. Examples: thrombolytics/fibrinolytics (tPA, streptokinase) which acutely elevate D-dimer; estrogen-containing drugs (oral contraceptives, HRT) and tamoxifen; some cancer therapies and erythropoiesis-stimulating agents; and drugs linked to heparin‑induced thrombosis. Any drug provoking thrombosis, inflammation, or fibrinolysis may raise D-dimer.
