Cytomegalovirus IgM-(CSF)

A negative CMV IgM result usually means there is no recent or active cytomegalovirus infection. If CMV IgG is positive, it indicates past infection and likely immunity; if IgG is also negative, it suggests no prior exposure. False negatives can occur early after exposure or in immunocompromised individuals, so repeat testing or PCR may be advised if clinical suspicion persists.

Cytomegalovirus (CMV) IgM positive means the immune system has produced IgM antibodies against CMV, indicating a recent or active infection or possible reactivation. IgM can appear early and sometimes persists, so results require correlation with symptoms, CMV IgG and IgG avidity testing to distinguish primary infection from past exposure. Evaluation is important in pregnancy due to congenital transmission risk; confirmatory testing recommended.

CMV detected in CSF (CMV CNS disease) is treated with antiviral therapy—first‑line intravenous ganciclovir (dose adjusted for renal function), often followed or transitioned to oral valganciclovir for step‑down. Severe or ganciclovir‑resistant cases may require foscarnet (sometimes combined). Treatment typically lasts several weeks with clinical and PCR monitoring; reduce immunosuppression and optimize HIV ART if applicable, and monitor blood counts and renal function.

CMV-negative generally means you have no evidence of past or current cytomegalovirus infection, which is good for most healthy people. However, it also means you’re susceptible to a primary infection—important for pregnant or immunocompromised individuals, since primary CMV can cause congenital infection or severe disease. Practice hygiene, consider repeat testing, and discuss prevention with your healthcare provider if concerned.

Kissing can transmit cytomegalovirus (CMV) because it spreads in saliva. If you’re pregnant or immunocompromised, avoid kissing someone known to have CMV—or avoid mouth-to-mouth contact with young children who commonly shed the virus. Healthy adults usually have mild or no symptoms, and prior immunity lowers but doesn’t eliminate risk. Practice hand hygiene, avoid sharing cups/utensils, and contact a healthcare provider if concerned after exposure.

About 40–60% of adults in developed countries are CMV‑negative (lack CMV antibodies), with rates varying by age and region. O negative blood is relatively uncommon: roughly 6–8% of people are O‑negative (around 7% in many Western populations). Seroprevalence tends to be higher in lower‑income countries and older adults.
