Contrast is usually not required for wrist CT when evaluating bones, fractures, or joint alignment; noncontrast CT gives excellent bony detail. Intravenous contrast may be added when assessing soft‑tissue masses, infection, inflammation, or suspected vascular injury. CT arthrography (intra‑articular contrast) can help evaluate cartilage and ligaments. The choice depends on the clinical question, allergies, and kidney function.

A CT scan of the wrist is an imaging test that uses X-rays and computer processing to produce detailed cross-sectional images of the bones and soft tissues. If contrast dye is required there may be additional time for injection and observation (sometimes up to an hour). Bring any referral and arrive a little early for paperwork.

For a wrist CT the arm is positioned so the wrist is centered in the scanner, usually with the forearm supported and the wrist in a neutral, non‑rotated position. The hand is commonly placed palm down (or neutral thumb‑up if needed) and immobilized with cushions or straps to prevent movement. Fingers may be slightly flexed; alignment along the scanner’s axis ensures accurate axial and 3D reconstructions.

CT scans without contrast are generally safe and noninvasive. They use ionizing radiation, so exposure is low but not zero; repeated scans increase cumulative lifetime risk. There’s no contrast-related allergy or kidney risk, but pregnant people and young children are more radiation-sensitive, so scans are done only when necessary. Discuss benefits and alternatives with your clinician to minimize unnecessary exposure.

For wrist problems, CT excels at detailed bone imaging—complex fractures, small fragments, and preoperative 3D planning—while MRI is superior for soft tissues: ligaments, TFCC, cartilage, bone marrow edema and occult fractures. CT uses ionizing radiation but is faster; MRI avoids radiation but is costlier, longer, and contraindicated for some implants. Choice depends on the suspected injury and clinical question.
