CT Scan Pituitary

A CT scan of the pituitary gland is a noninvasive X-ray imaging test that produces detailed cross-sectional images of the pituitary and surrounding skull base. It detects tumors, cysts, bone changes, hemorrhage, or abnormalities affecting hormone function. Often performed with thin slices and sometimes contrast dye, it helps plan treatment or guide further tests like MRI, which shows better soft-tissue detail.

CT head can detect larger pituitary tumors, bone changes, hemorrhage, or calcification, but small pituitary microadenomas are often missed. Contrast-enhanced CT improves visibility, yet MRI is the imaging modality of choice for detailed evaluation of the pituitary and nearby structures. If MRI isn’t possible, CT provides useful information but may require follow-up MRI for definitive assessment.

MRI is the preferred imaging for the pituitary gland because it offers superior soft‑tissue contrast and higher sensitivity for microadenomas, especially with a dynamic contrast‑enhanced thin‑slice pituitary protocol. CT is less sensitive for small pituitary lesions but useful for bone detail, calcification, acute hemorrhage, or when MRI is contraindicated or unavailable; CT also exposes the patient to ionizing radiation.

Magnetic resonance imaging (MRI) of the pituitary with gadolinium contrast—using thin, dedicated sellar and dynamic sequences—is the imaging modality of choice to evaluate the pituitary gland and nearby structures. It provides superior soft-tissue resolution to detect microadenomas, macroadenomas, cysts, hemorrhage, and stalk abnormalities. CT is reserved for bony detail or acute hemorrhage when MRI is contraindicated.

Early signs of a pituitary tumor often include persistent headaches and visual changes—especially loss of peripheral vision or blurred vision. Hormonal symptoms may appear: menstrual irregularities or infertility in women, decreased libido and erectile dysfunction in men, unexplained lactation (galactorrhea), fatigue, weight/appetite changes and mood swings. Larger tumors can cause nausea, vomiting or worsening headaches.

CT can detect larger pituitary prolactin-secreting tumors (macroadenomas) and bony changes, but it’s much less sensitive than MRI for small adenomas (microadenomas). Diagnosis relies first on raised serum prolactin; MRI with a dedicated pituitary protocol is the imaging gold standard. CT is an alternative when MRI is contraindicated or unavailable, but small prolactinomas may be missed.
