CT cisternography is an imaging test where iodinated contrast is injected into the cerebrospinal fluid via lumbar puncture, then CT scans outline CSF spaces. It helps localize CSF leaks, skull‑base or spinal defects, and certain cisternal abnormalities to guide treatment. Risks include headache, infection, allergic reaction to contrast, and radiation exposure.

Costs vary widely. In the United States, a CT cisternography typically costs about $1,000–$5,000 (private rates). In India, private centers charge roughly ₹4,000–₹25,000. In the UK it's usually covered by the NHS; private scans may cost £300–£1,200. Final price depends on facility, location, contrast use and physician fees—confirm with the hospital or insurer.

A normal CT cisternography shows symmetric, continuous contrast filling of the cerebral cisterns, subarachnoid spaces and ventricular system without focal defects, mass effect, or obstruction. There is no extravasation of contrast outside the subarachnoid space (no CSF leak), and the skull base and foramina appear intact. Ventricular size and sulcal pattern are within expected limits for the patient’s age.

CT cisternography is generally safe when done by experienced teams, but it involves a lumbar puncture and intrathecal contrast. Risks include headache, back pain, bleeding, infection, allergic reaction, rare seizures, CSF leak and radiation exposure. It’s usually avoided with raised intracranial pressure or severe contrast allergy. Benefits often outweigh risks for specific diagnoses; discuss individualized risks and alternatives with your physician.

A cerebrospinal fluid (CSF) leak can be serious. It causes low CSF pressure with severe positional headaches, nausea, neck stiffness, and sometimes hearing or vision changes. Open leaks increase risk of meningitis and brain infection. Small leaks may resolve with bed rest or an epidural blood patch; others need imaging and surgical repair. Seek prompt medical evaluation if suspected.

Duration depends on the type. MR cisternography is noninvasive and usually takes about 30–60 minutes. CT cisternography, including lumbar puncture and contrast, often requires 1–2 hours. Radioisotope (radionuclide) cisternography involves injection and multiple scans—immediate, ~6 and 24 hours and sometimes 48–72 hours—so the study can span 1–3 days with short imaging sessions.
