Celiac Profile

A celiac profile test is a set of blood tests used to screen for and help diagnose celiac disease. It typically includes anti-tissue transglutaminase (tTG) IgA, total serum IgA, anti-endomysial antibodies (EMA), and sometimes deamidated gliadin peptide (DGP) antibodies or HLA-DQ2/DQ8 genotyping. Accurate results require the patient to be consuming gluten; positive results often lead to intestinal biopsy for confirmation.

Four key markers used to diagnose celiac disease are: tissue transglutaminase IgA (tTG‑IgA), endomysial antibody IgA (EMA‑IgA), deamidated gliadin peptide antibodies (DGP—IgA and/or IgG), and total serum IgA level (to detect IgA deficiency that can affect test accuracy). These tests are interpreted with clinical features and, if needed, duodenal biopsy or HLA‑DQ2/DQ8 typing.

A positive celiac blood test indicates you have antibodies suggesting an immune reaction to gluten and raises strong suspicion for celiac disease. It isn’t definitive—confirmation usually requires clinical assessment, intestinal biopsy or genetic (HLA) testing and staying on gluten until testing. Positive results warrant gastroenterology referral, assessment for nutritional deficiencies and bone health, and guidance on a strict lifelong gluten-free diet if confirmed.

Five common signs and symptoms of celiac disease are chronic diarrhea and steatorrhea (fatty, foul‑smelling stools), abdominal pain with bloating, unintended weight loss or poor growth in children, iron‑deficiency anemia causing fatigue and pallor, and dermatitis herpetiformis — an intensely itchy, blistering skin rash often directly linked to gluten exposure.

Doctors confirm celiac disease with blood tests and intestinal biopsy. Initial serology checks tissue transglutaminase IgA (with total IgA) and sometimes deamidated gliadin antibodies; testing should be done while the patient is eating gluten. If serology is positive or unclear, an upper endoscopy with duodenal biopsies looks for villous atrophy and increased intraepithelial lymphocytes. HLA DQ2/DQ8 testing can help exclude celiac when inconclusive.

Celiac disease is not curable; it’s a lifelong autoimmune condition in which gluten damages the small intestine. The only effective treatment is a strict, lifelong gluten-free diet, which allows intestinal healing and symptom control for most people. Some individuals have refractory disease or complications and need specialist care. Regular medical follow-up and nutrient monitoring are important to prevent long-term problems.
