A CECT abdomen with rectal contrast is a contrast-enhanced CT scan of the abdomen and pelvis where iodinated contrast is given intravenously and additional contrast is introduced into the rectum. Rectal contrast distends and outlines the colon and rectal lumen, improving detection of perforation, leaks, fistulae, inflammation, abscesses and tumors. Preparation may include bowel cleansing; risks include contrast reactions and local discomfort.

A CECT scan is generally not painful. The CT itself is painless, though lying still may cause mild discomfort. The IV contrast can cause a brief sting, flushing or metallic taste; rare allergic reactions or kidney effects can occur. IV insertion may hurt slightly. Most people tolerate the procedure well—inform staff about allergies, pregnancy or kidney problems beforehand.

Rectal contrast is used to outline the colon and rectum during imaging to improve detection of leaks, perforations, fistulas, obstructions, inflammation, diverticula and tumors. It enhances visualization of the bowel lumen and wall, helps distinguish bowel from nearby structures, and clarifies anatomy after surgery or trauma. It complements intravenous/oral contrast to increase diagnostic accuracy for abdominal and pelvic conditions.

Fasting is often recommended before a contrast-enhanced CT (CECT) abdomen, mainly to reduce nausea/aspiration risk and to allow oral contrast to be given if needed. Typical instructions range from no solid food for about 2–6 hours; clear fluids and water are usually allowed. Requirements vary by facility and contrast type, so follow the imaging center’s specific instructions.

Which is better depends on the clinical need: colonoscopy is the gold standard for detecting and removing polyps and diagnosing mucosal disease, allowing biopsy and treatment. Contrast-enhanced CT is less sensitive for small polyps but better for assessing the bowel wall, extraluminal spread, obstruction, or when colonoscopy is contraindicated. For screening/definitive diagnosis, colonoscopy is preferred.

A contrast-enhanced CT (CE CT) shows structures in the scanned region. Commonly visualized organs include brain, heart, lungs, liver, spleen, pancreas, kidneys and adrenals; bowel and mesentery; major vessels (aorta, pulmonary and portal systems); lymph nodes, bones and soft tissues. Contrast improves assessment of organ perfusion, tumors, inflammation, vascular injury and active bleeding.
