Cardiolipin IgA Antibody

Cardiolipin IgA antibody is an autoantibody directed against cardiolipin, a phospholipid in cell membranes. It is one of the antiphospholipid antibodies linked to antiphospholipid syndrome, increasing risk of blood clots and pregnancy loss. IgA is measured less often than IgG/IgM but may be clinically relevant; persistent positivity (≥12 weeks) plus symptoms guides diagnosis. False positives can occur with infections.

A positive cardiolipin antibody test means antiphospholipid antibodies are present. This can indicate antiphospholipid syndrome, which raises risk of blood clots, recurrent miscarriage, stroke and low platelets. A single positive may be transient (infection, medications); diagnosis usually needs repeat testing after 12 weeks plus clinical assessment. Management and need for anticoagulation or specialist referral depend on antibody level, isotype and history.

Anticardiolipin IgA is generally reported as negative if below 20 U/mL, borderline at 20–40 U/mL, and positive when above 40 U/mL; higher values have greater clinical significance. Reference ranges vary by laboratory, so interpret with lab-specific cutoffs and clinical context. Persistent positivity should be confirmed by repeat testing at least 12 weeks apart for antiphospholipid syndrome evaluation.

Anticardiolipin antibodies are commonly found in antiphospholipid syndrome and systemic lupus erythematosus; they also occur with other autoimmune conditions (rheumatoid arthritis, Sjögren’s), certain infections (syphilis, HIV, hepatitis), some cancers, and drug‑induced or transient post‑infectious states. Clinically they’re associated with arterial/venous thrombosis, recurrent pregnancy loss and other pregnancy complications.

Anti-cardiolipin antibodies can increase risk of blood clots, pregnancy loss, and stroke when part of antiphospholipid syndrome, but a single positive test doesn’t confirm disease. Doctors require compatible symptoms and repeat testing (usually ≥12 weeks) before diagnosing. Cardiolipin is a signature phospholipid concentrated in the inner mitochondrial membrane, where it stabilizes respiratory chain complexes and supports energy production.
