Brain natriuretic peptide (BNP)

High B-type natriuretic peptide (BNP) indicates cardiac wall stretch from increased pressure or volume, most commonly heart failure or acute decompensation. Elevated BNP helps diagnose and gauge severity and prognosis. Levels can also rise with renal impairment, older age, pulmonary hypertension, acute coronary syndrome, or sepsis. Brain natriuretic peptide (BNP) is a hormone released mainly by ventricular heart muscle in response to increased stretch or pressure. It promotes natriuresis and diuresis, dilates blood vessels, and suppresses the renin–angiotensin–aldosterone system and sympathetic activity. These effects reduce blood volume and pressure, easing cardiac workload. Clinically, BNP levels help diagnose and monitor heart failure.

BNP is generally considered normal when under about 100 pg/mL in adults. For NT‑proBNP—an alternative assay with age‑adjusted thresholds—commonly used cutoffs are: <450 pg/mL for people under 50, <900 pg/mL for ages 50–75, and <1,800 pg/mL for over 75. Interpretation depends on symptoms, kidney function and assay; correlate clinically.

A BNP above 400 pg/mL is strongly suggestive of heart failure; levels between 100–400 pg/mL are indeterminate and require further clinical assessment; BNP under 100 pg/mL makes heart failure unlikely. For NT‑proBNP, >300 pg/mL suggests acute heart failure, with higher age‑adjusted cutoffs used for chronic evaluation. Clinical context and imaging confirm diagnosis.

High BNP signals heart strain and is treated by addressing the underlying cause—usually heart failure. Management includes diuretics to remove excess fluid; guideline‑directed medicines (ACE inhibitors/ARBs or ARNI, beta‑blockers, MRAs, SGLT2 inhibitors); blood‑pressure and ischemia control; and device therapy or IV treatments for acute cases. High BNP indicates heart strain and often suggests heart failure. It can signal an emergency when accompanied by severe breathlessness, chest pain, fainting, very low blood pressure, sudden swelling, or confusion — these require urgent evaluation and treatment.
