A normal (negative) anti‑dsDNA result is typically below the laboratory’s cutoff—commonly <30 IU/mL (some labs use <25–50 IU/mL). Values above the upper limit are considered positive; many labs designate 30–60 IU/mL as borderline and >60 IU/mL as clearly positive. Reference ranges and methods (ELISA, Farr, CLIFT) vary, so interpret results with the reporting lab and clinician.

DS DNA antibody positive means blood contains antibodies targeting double‑stranded DNA, reflecting an autoimmune response. It’s strongly associated with systemic lupus erythematosus (SLE), particularly active disease and kidney involvement (lupus nephritis). Detection helps support SLE diagnosis and monitor disease activity; levels can rise with flares and fall with treatment. Tests include ELISA, immunofluorescence, or Farr assay.

Low titer anti‑double‑stranded DNA (anti‑dsDNA) means small amounts of antibodies against native double‑stranded DNA detected in blood. Low levels are less specific for active systemic lupus erythematosus and can occur in healthy people or other conditions. Clinical context, repeat testing, ANA results and complement levels guide interpretation; rising titers or high levels are more suggestive of active lupus or kidney involvement, so follow‑up is advised.

No single test alone confirms systemic lupus erythematosus. Diagnosis combines clinical features with laboratory testing: an ANA screen (high sensitivity) followed by specific antibodies such as anti‑double‑stranded DNA and anti‑Smith (more specific). Low complement levels, raised inflammatory markers, urine tests for kidney involvement and, when needed, tissue biopsy (eg, renal biopsy) complete the workup.

A persistently positive antinuclear antibody (ANA) test is the single most sensitive indicator of systemic lupus, appearing in the vast majority of patients. It isn’t disease-specific, so further tests (anti‑dsDNA/anti‑Sm) and clinical features guide diagnosis. The classic malar “butterfly” rash across the cheeks and nose remains the most characteristic visible sign.

An ANA titer of 1:320 is generally considered moderately high and typically warrants further evaluation. It raises the possibility of an autoimmune condition but is not diagnostic alone—clinical symptoms, exam findings, and more specific antibody tests (eg, anti‑dsDNA, ENA panel) are needed.
