Aldolase

High aldolase usually indicates muscle cell damage—seen with inflammatory myositis, muscular dystrophy, rhabdomyolysis, recent injury or intense exercise—and less commonly liver disease or hemolysis. It’s a nonspecific marker, so doctors compare it with symptoms and other tests (creatine kinase, liver enzymes, imaging, autoimmune panels). Repeat testing and specialist evaluation may be needed to determine the cause and guide treatment.

Aldolase is an enzyme in glycolysis that splits fructose‑1,6‑bisphosphate into triose phosphates. Clinically, blood aldolase levels are measured to assess muscle injury, inflammatory myopathies, muscular dystrophy and some liver disorders; levels can help diagnose and monitor disease activity and treatment response. It’s a general marker of muscle cell damage rather than a disease‑specific test.

Aldolase A is a glycolytic enzyme that cleaves fructose-1,6-bisphosphate into glyceraldehyde-3-phosphate and dihydroxyacetone phosphate, enabling ATP generation during glycolysis. Predominant in muscle and red blood cells, it supports anaerobic energy production and can operate reversibly in gluconeogenesis. Aldolase A also aids cytoskeletal interactions and cell metabolism; deficiency may cause impaired energy supply, hemolytic anemia, and muscle weakness.

A low aldolase result usually isn’t a specific problem. Aldolase is a muscle and liver enzyme; low levels often reflect low muscle mass, lab variability, or technical issues rather than disease. Results are interpreted with symptoms and other tests (e.g., CK, liver panel). If you have unexplained symptoms or concerns, repeat testing or discuss interpretation with your clinician for appropriate follow-up.

Aldolase is a glycolytic enzyme whose blood level rises with muscle injury and liver disease. Although mild elevations can occur with some cancers (e.g., hepatocellular or ovarian), aldolase is neither specific nor sensitive as a tumor marker. It is not routinely used for cancer screening or diagnosis and has limited clinical utility; results need correlation with clinical context and other tests.

Aldolase itself is an enzyme; abnormal levels reflect underlying problems. Elevated aldolase from muscle disease causes muscle weakness, cramps, pain, fatigue, exercise intolerance, difficulty walking, swallowing or breathing, and sometimes dark urine. Aldolase B defects (hereditary fructose intolerance) cause hypoglycemia, vomiting, abdominal pain, jaundice and poor growth. Some genetic aldolase deficiencies can also cause hemolytic anemia.
