Absolute Neutrophil Count

High absolute neutrophils (neutrophilia) means an elevated neutrophil white blood cell count, often indicating acute bacterial infection, inflammation, tissue injury, stress, or steroid use. Less commonly it reflects smoking, pregnancy, or bone marrow disorders. Mild, temporary rises can be benign; persistent or very high counts need clinical evaluation and further tests to determine the cause and guide treatment.

What happens if your absolute neutrophils are low? Increased risk of bacterial and fungal infections; even minor infections can become severe. Symptoms include fever, sore throat, mouth ulcers, chills, and frequent infections. Severity depends on the absolute neutrophil count (mild ~1,000–1,500/µL, moderate 500–1,000/µL, severe <500/µL). Management includes infection prevention, prompt medical evaluation for fever, possible antibiotics, and growth factors or medication changes.

ANC = WBC count (cells/µL) × (percent neutrophils + percent bands)/100. For example, WBC 5,000/µL with 60% neutrophils and 5% bands: ANC = 5,000 × 0.65 = 3,250/µL. Interpretation: ANC ≥1,500/µL normal; 1,000–1,500 mild neutropenia; 500–1,000 moderate; <500 severe. Use CBC differential percentages and report ANC in cells/µL; consult clinician if low.

Raised neutrophil counts in pregnancy are often a normal, pregnancy‑related change—especially in the second and third trimesters and during labor—reflecting immune adaptation. However, marked neutrophilia can signal infection, inflammation, stress, or medication effects, and may warrant further tests if accompanied by fever, pain, or abnormal fetal signs. Your clinician will interpret levels alongside symptoms and other blood results.

High neutrophil counts (neutrophilia) can occur with myeloproliferative neoplasms—chronic myeloid leukemia and chronic neutrophilic leukemia—and with some solid tumors that cause paraneoplastic cytokine release: notably lung, renal cell, hepatocellular, colorectal, pancreatic, gastric and ovarian cancers. Tumor secretion of G‑CSF/GM‑CSF often drives the neutrophilia; evaluation should exclude infection or inflammation.

No vitamin deficiency typically causes elevated neutrophils. Deficiencies of vitamin B12 and folate more commonly produce neutropenia rather than neutrophilia. High neutrophil counts are usually reactive—caused by acute bacterial infection, inflammation, corticosteroids, smoking, stress, or blood cancers—so unexplained neutrophilia warrants clinical review, medication check, and further testing.
