An upper abdominal ultrasound assesses the liver (size, texture, cysts, tumors, fatty change), gallbladder and bile ducts (stones, inflammation, dilation), pancreas (masses, inflammation), spleen (size, lesions), upper poles of the kidneys and nearby blood vessels including the abdominal aorta, and looks for fluid collections, masses or signs of obstruction. It helps diagnose stones, infections, enlargement, tumors and aneurysms.

For the best 3D fetal images, lie semi‑recumbent with a slight left tilt so the transducer can access the abdomen comfortably. Optimal fetal position is face anterior and turned toward the probe, with hands away from the face and adequate amniotic fluid around the head. Best results are usually between about 26–32 weeks; gentle maternal movement or drinking a sweet drink can encourage repositioning.

The best scan depends on the clinical question. For initial assessment of liver, gallbladder and biliary disease, abdominal ultrasound is the first-line test. For acute pain, trauma, suspected malignancy or detailed organ evaluation, contrast-enhanced CT abdomen is preferred. MRI (and MRCP) is best for detailed liver lesion characterization or pancreatic/biliary duct imaging. Your doctor will choose based on symptoms and findings.

3D ultrasound provides three-dimensional images useful for detecting fetal structural anomalies (facial defects like cleft lip, limb and skeletal malformations, neural tube and spinal defects), some cardiac and abdominal abnormalities, placental position and cord issues, and growth problems. Outside pregnancy it helps evaluate uterine and ovarian abnormalities (fibroids, congenital uterine anomalies), guiding surgical planning and infertility assessment.

An upper‑abdominal (transabdominal) ultrasound is very safe and noninvasive, using no ionizing radiation. Risks are minimal: transient discomfort from probe pressure, rare skin irritation or allergic reaction to the gel (or to contrast agents if used), and possible inconclusive images (due to bowel gas or body habitus) that require repeat or alternative testing. No known long‑term adverse effects.

Abdominal ultrasound can detect liver cancers (primary hepatocellular carcinoma and metastases), kidney cancer (renal cell carcinoma), gallbladder and bile-duct tumours (cholangiocarcinoma), pancreatic masses (sometimes), splenic lesions and lymphoma, adrenal masses, and pelvic tumours such as ovarian masses. Sensitivity varies by organ and lesion size; suspicious findings usually need CT, MRI or biopsy for confirmation.
