A 2D ultrasound biophysical assessment in pregnancy uses two‑dimensional imaging to evaluate fetal well‑being. It visualizes fetal movements, breathing motions, muscle tone and measures amniotic fluid volume as part of a biophysical profile (BPP). Combined with heart‑rate monitoring when needed, it helps detect fetal distress, growth or placental issues and guides management in high‑risk pregnancies.

A biophysical ultrasound (biophysical profile) is a noninvasive test combining ultrasound with fetal heart monitoring to assess fetal well‑being late in pregnancy. It evaluates fetal breathing, body movements, muscle tone, amniotic fluid volume and heart‑rate reactivity, each scored to give an overall risk assessment. It's used mainly in high‑risk pregnancies to detect possible fetal distress and guide management.

A good biophysical profile (BPP) score is eight to ten out of ten, indicating fetal well‑being. The BPP combines a nonstress test and four ultrasound measures: fetal breathing, gross body movements, muscle tone and amniotic fluid volume. A score of six is equivocal and needs close monitoring or further testing; scores of four or less are concerning and may prompt urgent evaluation or delivery.

The five categories (components) of the biophysical profile are fetal breathing movements, gross fetal movements, fetal tone, nonstress test assessment of fetal heart rate reactivity, and amniotic fluid volume. Each component is scored to evaluate fetal well‑being, with higher total scores indicating better oxygenation and lower risk of compromise, to guide management.

A 2D ultrasound produces real-time, non-invasive, black-and-white cross-sectional images of internal organs and tissues. It’s used in pregnancy to confirm viability, estimate gestational age, monitor fetal growth and anatomy, check placenta and amniotic fluid, and detect anomalies. Outside obstetrics it evaluates organs (heart, liver, kidneys), detects masses or fluid, and guides procedures like biopsies or drainages.

A 32‑week ultrasound assesses fetal growth and estimated weight, confirms fetal presentation (head or breech), evaluates placental position and function, and measures amniotic fluid volume. It checks fetal anatomy, movement and heart activity and—if indicated—uses Doppler studies or a biophysical profile to assess blood flow and well‑being. Findings help detect growth restriction or other problems and guide monitoring and delivery planning.
